Payment will be requested at the time of service for all services which are non-covered or determined to be patient’s responsibility, including co-payments and deductibles.
We will kindly reschedule your appointment if you do not have your co-payment. Methods of payment accepted include cash, debit, Mastercard and Visa. We do not accept personal checks.
**IT IS YOUR RESPONSIBILITY TO BE AWARE OF YOUR INSURANCE BENEFITS. ANY SERVICE NOT COVERED WILL BE PATIENT RESPONSIBILTY.**
Agave Family Health, PLLC participates with most major insurance carriers. As a courtesy to our patients, we will file insurance claims for those insurances with which we participate.
The agreement of the insurance company to pay for medical services is between you and your carrier.
If the patient fails to provide us with all the correct information, they are financially responsible for the office visit charges.
Please remember, any amount not covered by insurance is ultimately the patient’s responsibility.

We recommend that all patients confirm coverage with your insurance company before you arrive for your appointment.
Delinquent accounts over 60 days will be sent to collections for processing. At which point all collection and legal fees of 50% the total balance will be added to the resposibility of the patient. Please contact our billing department to make payment arrangements before that happens.
